It is important that to note that all of the information that you provide with these evaluations will be held in strict confidence and will not be shared with the coaching staff. This information will be used to help the Board and the Coaches Selection Committtee prepare for the hockey season ahead. Most importantly, it will help to create the best experience for you and your child.
Please complete and either email it back to me, or drop off a copy at the CMHA mail box in Eddie Bush and Central. Alternatively the form can be filled out on our website under the Coach Evaluation tab. A copy of this survey will automatically be sent to me.
